  This is an online e-log book to discuss our patient's de-identified health data shared after taking his/her/guardian's informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.   A 51 yr old male patient presented to Opd with cheifcomplaints of pedal edema and facial puffiness since 2 months.   History of present illness :       Patient was apparently asymptomatic 1 yr back then he developed pedal edema and facial puffiness for which he visited hospital and diagnosed as renal dysfunction and he was on medication and dialysis also done once and symptoms subsided . Then he stopped using medications and symptoms recurred and then   he  visited our hospital and investigations are done and diagnosed as chronic kidney failure.And kept on maintenance dailysis twice a week.     Patient 

 1.Anatomical  and  etiological localisation of hemiparesis and it's further management ? 2. Etiology pathogenesis  clinical features management and complications of pancreatitis?  3. Dengue fever clinical features and complications   ?   4. Cushing syndrome ?    5. Mandibular advancement device? Not attempted 6. Pulmonary edema?  7.  Rheumatoid arthritis ?    8.  Leptospirosis? 9.  Heart failure? 10 . Ascites? 11.pyrexia of unknown origin ?  12. Drug induced liver failure?  13 . Evaluation of lower back pain? 14. Renal artery stenosis?     15. Acute kidney injury? 16. History of present illness:  Patient was apparently asymptomatic 1 week back then he developed high fever   then he visited a nearer medical care centre and diagnosed as typhoid and treatment is given and for 2 days he was alright. And later after two days again he developed fever  and chills during night time and serve weakness that he  was not able to take food then  he visited nearer hospital and diagnosed as decreased platelet count with hyperpyrexia  then referre
